Introduction
Aneurysm repair is a critical surgical procedure performed to treat an aneurysm, which is a weakened, bulging area in the wall of a blood vessel. Aneurysms can occur anywhere in the body, but they are most common and dangerous in the aorta (the body’s main artery) or the brain. If an aneurysm continues to grow, the vessel wall becomes increasingly thin and risks rupturing, which causes massive internal bleeding and is often fatal. The goal of aneurysm repair is to reinforce the weakened artery, prevent rupture, and restore normal, safe blood flow.

Surgical Procedure
There are two main approaches to repairing an aneurysm, depending on its size, location, and the patient’s overall health: open surgical repair and endovascular aneurysm repair (EVAR).
In open surgical repair, the patient is placed under general anesthesia. The surgeon makes a large incision to directly access the aneurysm (e.g., in the abdomen for an abdominal aortic aneurysm). The blood flow is temporarily clamped above and below the aneurysm. The surgeon then cuts open the bulging section of the artery and sews a synthetic fabric tube (a graft) into place to replace the weakened segment. The artery wall is often wrapped around the graft for extra support, and the incision is closed.
In endovascular repair, a minimally invasive technique, small incisions are made in the groin to access the femoral arteries. A catheter carrying a collapsed stent graft (a fabric tube supported by a metal wire mesh) is guided through the blood vessels to the site of the aneurysm using X-ray guidance. Once in position, the stent graft is expanded, creating a new, strong path for blood to flow through, effectively bypassing the weakened aneurysm wall.

Surgical Risks
Aneurysm repair is a major procedure with significant risks, including adverse reactions to general anesthesia, severe bleeding, and blood clots. Open surgery generally carries higher risks than endovascular repair.

General Complications
Common complications include:
– Pain and discomfort at the incision sites.
– Fatigue and weakness during the recovery period.
– Changes in bowel habits or temporary loss of appetite (especially after abdominal surgery).
– Swelling in the groin or legs (after endovascular repair).

Rare but Serious Complications
Serious complications require immediate medical intervention:
– Endoleak (specific to EVAR): Blood leaking back into the aneurysm sac around the stent graft, which may require further procedures.
– Graft infection or thrombosis (blood clot inside the graft).
– Damage to surrounding blood vessels or organs.
– Kidney damage, particularly if the aneurysm is near the renal arteries.
– Spinal cord ischemia (reduced blood flow to the spinal cord), which can lead to paralysis in rare cases of complex aortic repairs.

Pre-operative Preparation
– Comprehensive Imaging: CT scans or MRIs are essential to precisely map the size and location of the aneurysm.
– Medical Evaluation: Extensive blood tests, ECG, and cardiac evaluations are required to ensure the patient can withstand the surgery.
– Fasting: Complete fasting for 8 to 12 hours prior to the procedure.
– Medication Management: Blood thinners and certain other medications must be stopped several days in advance.

Post-operative Care
Following open surgery, patients usually spend a few days in the ICU and have a total hospital stay of 5 to 10 days. Endovascular repair patients may only stay 1 to 3 days. Vital signs, kidney function, and blood flow to the legs are closely monitored. Pain relief and early, gentle movement are encouraged to aid recovery.

Wound Care
– Keep incisions clean and dry. Follow instructions on showering.
– Do not soak in baths or pools until fully healed.
– Monitor for signs of infection (redness, warmth, drainage) or significant swelling at the incision sites.

Diet
– Resume a normal diet gradually. A heart-healthy diet low in saturated fats and sodium is crucial for long-term cardiovascular health.
– Stay well-hydrated to support kidney function, especially if contrast dye was used during the procedure.

Post-discharge Notes
Recovery from open surgery can take 4 to 8 weeks, while EVAR recovery is typically 1 to 2 weeks.
– Strictly avoid heavy lifting and strenuous activities until cleared by the surgeon.
– Do not smoke, as smoking significantly increases the risk of further vascular disease.
– Control blood pressure diligently, as high blood pressure puts stress on the repaired artery.
– Attend all follow-up imaging appointments (usually CT scans or ultrasounds) to ensure the graft remains in place and the aneurysm is not growing.
