Introduction
Total hip replacement, or total hip arthroplasty, is a highly successful surgical procedure designed to replace a damaged or diseased hip joint with an artificial implant (prosthesis). The hip is a ball-and-socket joint, and conditions such as severe osteoarthritis, rheumatoid arthritis, osteonecrosis (loss of blood supply to the bone), or a severe hip fracture can wear down the cartilage and bone, causing intense pain and restricted mobility. When daily activities like walking, putting on shoes, or sleeping become overwhelmingly difficult despite conservative treatments, a hip replacement is often recommended. The procedure aims to relieve pain, restore normal joint function, and significantly enhance the patient’s quality of life. Modern artificial hip joints are highly durable and can last for decades.

Surgical Procedure
The surgery is performed under general anesthesia or regional anesthesia (such as a spinal or epidural block).
The surgeon makes an incision over the side or front of the hip. There are different surgical approaches (anterior, posterior, or lateral), and the choice depends on the patient’s anatomy and the surgeon’s expertise. The surgeon carefully moves aside the muscles and tissues to expose the hip joint.
The damaged femoral head (the “ball” at the top of the thighbone) is removed and replaced with a metal or ceramic stem that is inserted into the hollow center of the femur. A metal or ceramic ball is then placed on the upper part of the stem. Next, the surgeon removes the damaged cartilage surface from the acetabulum (the “socket” in the pelvis) and presses a metal socket into place. A plastic, ceramic, or metal spacer is inserted between the new ball and socket to ensure a smooth gliding surface. The surgeon tests the joint for stability and range of motion before closing the incision and applying a dressing.

Surgical Risks
As a major orthopedic surgery, total hip replacement carries inherent risks, including adverse reactions to anesthesia, significant bleeding, and the potential for blood clots in the legs (DVT) or lungs (PE), which are actively managed with blood thinners and compression devices post-surgery.

General Complications
Common, manageable complications following surgery include:
– Post-operative pain and swelling around the hip and thigh.
– Leg length discrepancy: Sometimes, the operated leg may feel slightly longer or shorter than the other, though this usually resolves as muscles adapt.
– Stiffness in the hip joint, requiring dedicated physical therapy.
– Bruising that may extend down the leg.

Rare but Serious Complications
Serious complications are rare but require immediate attention:
– Hip Dislocation: The artificial ball can pop out of the socket, especially in the first few months while tissues are healing. Certain movements must be avoided to prevent this.
– Infection: Superficial wound infections or deep infections around the prosthesis can occur, potentially requiring IV antibiotics or further surgery.
– Implant Loosening: Over many years, the implant may loosen from the bone, necessitating a revision surgery.
– Nerve or blood vessel injury during the procedure, causing numbness or weakness in the leg.

Pre-operative Preparation
– Medical Clearance: A complete physical examination, blood tests, and an ECG are required.
– Dental Checkup: It is often recommended to complete any major dental work before surgery to prevent bacteria from entering the bloodstream and infecting the new joint.
– Home Modification: Prepare your living space by removing rugs, arranging items at waist level, and acquiring assistive devices like a raised toilet seat or a grabber tool.
– Fasting: No food or drink for 8 to 12 hours before the procedure.

Post-operative Care
Patients usually stay in the hospital for 1 to 2 days, though some may qualify for outpatient surgery. Pain management is provided via medications. Early mobilization is critical; physical therapists will help patients stand and walk with a walker or crutches on the day of or the day after surgery. They will also teach specific “hip precautions” (movements to avoid, such as crossing legs or bending past 90 degrees) to prevent dislocation, depending on the surgical approach used.

Wound Care
– Keep the incision clean and dry. Follow instructions regarding showering; sponge baths may be necessary initially.
– Do not submerge the hip in water (baths, pools) until fully healed.
– Monitor for signs of infection: excessive redness, swelling, warmth, or drainage.

Diet
A balanced diet is essential for healing. High-fiber foods and plenty of fluids are crucial to prevent constipation, a common side effect of narcotic pain relievers and reduced mobility.

Post-discharge Notes
Recovery requires adherence to rehabilitation exercises.
– Patients typically transition from a walker to a cane within a few weeks.
– Driving can usually resume in 4 to 6 weeks, provided the patient is off strong pain medications and has regained quick reaction times.
– Avoid high-impact sports (running, basketball) permanently. Low-impact activities like swimming, walking, and golf are excellent once cleared by the doctor.
– Seek immediate medical help for chest pain, shortness of breath, severe calf pain, or a sudden increase in hip pain.
